If you have been referred to R. Kwok, MD, for pain management services,  there are a few forms  you will need to download and complete prior to your appointment. These forms can be found under the drop-down box under "Pain Management." Please print out the form below (click on the link), complete it prior to your appointment, and bring it to your appointment.
